It is ideal for children to have medical forms completed for their first day of care. If you are not able to complete all medical forms by the first day please provide a doctor’s note from your physician stating the date of your child’s future doctor’s appointment. The date of the appointment, may not be later than 20 calendar days following the date the child was temporarily admitted in care. All medical forms must be completed within 3 business days of the appointment noted on the physician’s slip
